India’s healthcare system has undergone a major transformation over the past 12 years, driven by large-scale policy reforms, infrastructure investments and digital innovation aimed at improving healthcare access in India and strengthening health security for millions. These reforms have significantly improved affordability, accessibility and quality of healthcare services, particularly for rural and economically vulnerable populations. At the centre of this transformation is the flagship programme Ayushman Bharat, one of the world’s largest publicly funded health insurance initiatives. Under the Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB-PMJAY), nearly 12 crore families are eligible for annual health coverage of up to Rs. 5 lakh (US$ 5,234.51) per family. This initiative provides financial protection against catastrophic health expenses, which historically pushed millions of households into poverty due to high medical costs. Since its launch in 2018, the scheme has achieved remarkable milestones. More than 44.14 crore Ayushman cards have been issued across the country, ensuring that beneficiaries can access cashless treatment at empanelled hospitals. Additionally, over 12.03 crore hospital admissions have been supported under the scheme, highlighting its large-scale impact on healthcare delivery. The programme has also financed treatments worth more than Rs. 1.80 lakh crore (US$ 18.84 billion), demonstrating its critical role in reducing out-of-pocket healthcare expenditure. A key component of this transformation is the expansion of grassroots healthcare infrastructure through more than 1.86 lakh Ayushman Arogya Mandirs. These health and wellness centres serve as the first point of contact for millions of citizens, offering primary healthcare services, preventive care, maternal and child health services, diagnostics and basic treatment facilities. Collectively, these centres have recorded over 540 crore patient visits, reflecting their importance in strengthening primary healthcare delivery in India. These initiatives have also contributed to improvements in maternal and child health outcomes, immunisation coverage and the management of communicable and non-communicable diseases. By focusing on preventive healthcare and early diagnosis, the system is gradually reducing the burden on secondary and tertiary healthcare facilities. Another major advancement in India’s healthcare reform journey is the rapid adoption of digital healthcare in India. The Ayushman Bharat Digital Mission (ABDM) has created a unified digital health ecosystem that connects patients, healthcare providers and institutions through secure digital health records. With more than 20.49 crore registrations, this system is transforming how health data is stored, accessed and shared across the healthcare network. The introduction of digital platforms like the eSanjeevani telemedicine service has further revolutionised healthcare delivery, especially in remote and rural areas. Since its launch in 2019, eSanjeevani has facilitated over 47 crore teleconsultations, enabling patients to access specialist doctors without the need for physical travel. This has significantly improved healthcare accessibility in underserved regions and reduced the burden on urban hospitals. To strengthen long-term healthcare resilience, the government has launched the Pradhan Mantri Ayushman Bharat Health Infrastructure Mission (PM-ABHIM) with an outlay of Rs. 64,180 crore (US$ 6.72 billion). This mission focuses on building robust healthcare infrastructure, including advanced laboratories, critical care units, infectious disease control systems and improved surveillance mechanisms. The goal is to enhance India’s preparedness for future health emergencies and pandemics. In addition to infrastructure development, affordability has been a key focus of healthcare reforms. The expansion of Jan Aushadhi Kendras, which provide affordable generic medicines, has helped reduce treatment costs for millions of patients. These centres ensure that essential medicines are available at significantly lower prices compared to branded alternatives, making healthcare more accessible to low-income households. Free dialysis services and other government-supported treatment programmes have further strengthened the healthcare safety net. These initiatives ensure that patients suffering from chronic conditions receive continuous care without facing excessive financial burden. The overall impact of these reforms has been the strengthening of India’s universal health coverage goals, ensuring that more citizens have access to quality healthcare services without financial hardship. The integration of insurance coverage, digital health systems and infrastructure development has created a more coordinated and efficient healthcare ecosystem. The expansion of healthcare services has also improved public trust in government healthcare facilities. With better infrastructure, increased availability of doctors and improved service delivery, more people are now relying on public healthcare systems for treatment and preventive care. Another important aspect of India’s healthcare transformation is its focus on health system resilience. The COVID-19 pandemic highlighted the importance of strong healthcare infrastructure and digital readiness. In response, India has accelerated investments in critical care facilities, disease surveillance systems and emergency response mechanisms. Together, these reforms support India’s long-term vision of achieving universal health coverage in India and becoming a developed nation by 2047. By combining technology, infrastructure and inclusive policy design, the healthcare system is evolving into a more equitable and sustainable model. In conclusion, India’s healthcare reforms represent a comprehensive transformation that spans insurance coverage, digital innovation, infrastructure expansion and affordability initiatives. Programmes like Ayushman Bharat, ABDM, eSanjeevani and PM-ABHIM have collectively strengthened healthcare access and financial protection for millions of citizens. As these initiatives continue to expand, India is steadily moving towards a future where quality healthcare is not a privilege but a fundamental right accessible to every citizen, ensuring stronger health security, affordability and inclusivity across the nation.
