The Outbreak and Response

According to preliminary investigations, the likely source of the outbreak is the raw slivered onions served on the Quarter Pounders. Although the CDC and McDonald’s are scrutinizing both the onions and beef patties, health officials consider the beef patties less likely to be the culprit, given federal meat testing requirements and McDonald’s cooking protocols designed to eliminate harmful bacteria. Nevertheless, McDonald's quickly responded to the outbreak by removing the Quarter Pounder from the menu at approximately one-fifth of its 14,000 locations in the United States, particularly affecting outlets in states such as Colorado, Kansas, Utah, Wyoming, and various parts of Idaho, Iowa, Missouri, Montana, Nebraska, Nevada, New Mexico, and Oklahoma.

Joe Erlinger, President of McDonald's USA, acknowledged the need to restore public trust, stating that the company fully expects to see more cases emerge as the investigation unfolds. "McDonald's has moved rather quickly to take action to hopefully prevent as many cases as possible," said CDC spokesman Tom Skinner, indicating the urgency of addressing the outbreak.

Symptoms and Risks

Symptoms of E. coli poisoning typically manifest within one to two days after consuming contaminated food and may include fever, vomiting, diarrhoea, particularly bloody diarrhoea, and signs of dehydration, such as little or no urination, increased thirst, and dizziness. The infection can lead to serious kidney injuries, especially in children under five years of age, necessitating immediate medical attention.

In the United States, the strain of E. coli implicated in this outbreak accounts for approximately 74,000 infections annually, resulting in over 2,000 hospitalizations and 61 deaths each year. Fortunately, recent data indicates that E. coli infections have declined in 2023, with cases of severe kidney injury remaining stable.

Food poisoning affects at least 48 million Americans each year, with around 128,000 hospitalizations and 3,000 deaths attributed to various pathogens. While E. coli is a notable cause, it is not the only one. The CDC reports that norovirus leads to approximately 19 to 21 million cases of food poisoning annually, while Salmonella causes about 1.3 million illnesses. Although Listeria results in fewer infections (around 1,600 each year), it accounts for a significant number of fatalities, with about 260 deaths annually.
